Your Hospital Registration Checklist

No one likes being admitted to a hospital. It’s a stressful situation for anyone. The last thing you need to worry about is the registration process. So use this handy checklist to make sure your next admission is simple, fast and easy.

What to bring for hospital registration or pre-admission

• All active medical insurance cards

• Social Security card

• Picture ID (a driver’s license, for example)

• Any Advanced Directives such as durable power of attorney, living will or healthcare proxy, a document that gives another person authority to make medical decisions if you are unable to.

IMPORTANT REMINDER: If your medical insurance requires pre-certification, contact your insurance carrier or make sure your physician has obtained the necessary authorization and bring this with you to registration.

What to bring the day you are being admitted to the hospital

• A list of your medications that includes dosage, time the medication is taken, and any allergies

• Personal items such as a robe, slippers and toiletries

• Glasses

• Hearing aids
